Abstract

Epidural labor analgesia aims to provide effective medical services to alleviate labor pain in parturients, while adhering to the principles of voluntary participation and clinical safety. In 2018, Peking Union Medical College Hospital(PUMCH)became one of the first pilot units for labor analgesia in China, and has achieved satisfactory results in high-quality development of labor analgesia. This article mainly introduces the achievements and experience of labor analgesia at PUMCH, including: (1) prioritizing maternal and infant safety, arranging personnel rationally, and developing standardized treatment processes through multidisciplinary collaboration to ensure safe and comfortable childbirth; (2) leveraging the hospital's comprehensive capabilities in emergency treatment, and improving collaborative rescue plans for critically ill parturients and newborns; (3) implementing advanced teaching methods to effectively train and conduct simulated drills for labor analgesia and rescue of critically ill parturients; (4) conducting patient education and informative lectures to help parturients acquire a scientific understanding of labor analgesia. We hope that this experience can provide reference and inspiration for other hospitals.

32. Engineering Atomic Ag1–N6Sites with Enhanced Performance of Eradication Drug-Resistant Bacteria over Visible-Light-Driven Antibacterial Membrane

Abstract

Utilizing visible light for water disinfection is a more convenient, safe, and practical alternative to ultraviolet-light sterilization. Herein, we developed silver (Ag) single-atom anchored g-C3N4(P-CN) nanosheets (Ag1/CN) and then utilized a spin-coating method to fabricate the Ag1/CN-based-membrane for effective antibacterial performance in natural water and domestic wastewater. The incorporated Ag single atom formed a Ag1–N6motif, which increased the charge density around the N atoms, resulting in a built-in electric field ∼17.2 times stronger than that of pure P-CN and optimizing the dynamics of reactive oxygen species (ROS) production. Additionally, the Ag1–N6motif inhibited the release of Ag ions, ensuring good biocompatibility. Based on the first-principles calculation, the adsorption energy of O2on the Ag1/CN (−0.32 eV) was lower than that of P-CN (−0.07 eV), indicating that loaded Ag single atom can lower the energy barrier for O2activation, generating extra *OH radicals that cooperated with *O2–to effectively neutralize bacteria. As a result, the Ag1/CN powder-catalyst with the concentration of 30 ppm demonstrated a 99.9% antibacterial efficiency against drug-resistant bacteria (Escherichia coli, Staphylococcus aureus, kanamycin-resistant Escherichia coli, and methicillin-resistant Staphylococcus aureus) under visible-light irradiation for 4 h. This efficacy was 24.8 times higher than that of the P-CN powder catalyst. Moreover, the Ag1/CN-based-membrane can maintain a 99.9% bactericidal efficiency for natural water and domestic wastewater treatment using a homemade flow device, demonstrating its potential for water disinfection. Notably, the visible-light-driven antibacterial efficiency of the Ag1/CN catalyst outperformed the majority of the reported g-C3N4-based catalysts/membranes.

Abstract

Objective: With a global incidence of more than 10%, preterm birth (PTB) remains a significant concern. The vaginal microbiome strongly influences the well-being of the female reproductive tract. This study examines the correlation between changes in Lactobacillus vaginal microbiota and the PTB risk. Materials and Methods: A thorough search of PubMed, Web of Science, Cochrane Library, and EMBASE was conducted to locate studies that examined the association between changes in Lactobacillus vaginal microbiota and the risk of PTB from January 1, 2010, to January 30, 2023. The risk of PTB was determined by calculating odds ratios (ORs) with 95% confidence intervals (CIs). Results: In our analysis, there were 11 studies with 1577 pregnant women. The findings revealed a significant negative correlation between higher Lactobacillus abundance and the PTB risk (OR = 0.49, 95% CI: 0.29–0.84, p = 0.009 < 0.05). Similarly, the four individual dominant species, Lactobacillus crispatus (OR = 0.3, 95% CI: 0.14–0.67, p = 0.003 < 0.05), Lactobacillus gasseri (OR = 0.34, 95% CI: 0.17–0.69, p = 0.003 < 0.05), Lactobacillus iners (OR = 0.68, 95% CI: 0.49–0.93, p = 0.016 < 0.05), and Lactobacillus jensenii (OR = 0.43, 95% CI: 0.21–0.89, p = 0.024 < 0.05), were also negatively associated with the PTB risk. The risk of Lactobacillus for PTB was significant in both America (OR = 0.67; 95% CI: 0.50–0.92) and Asia (OR = 0.20; 95% CI: 0.09–0.47), whereas no significant risk was found in Europe (OR = 0.49; 95% CI: 0.11–2.15). Conclusions: Our study demonstrated that the abundance of Lactobacillus and the four dominant individual species (L. crispatus, L. jensenii, L. iners, and L. gasseri) were significantly and negatively associated with the PTB risk. Abstract

In common plasmonic configurations, energy loss and field enhancement are mutually restricted. In a vast majority of cases, high confinement goes together with high loss, which is a serious limitation for some applications. In an attempt of breaking this rule, which holds true for surface plasmon polariton (SPP) resonators, a multilayer trench grating microstructure with an asymmetric waveguide is considered. It supports both Fabry-Perot (FP) and cavity modes, whose hybridization exhibits unusual properties. The electric field enhancement was modulated by regulating the corresponding absorption and radiation quality factors. At the same time, energy loss was reduced, which is fundamentally ascribed to the mutual recycling of radiation energy between FP and cavity resonators. The maximum total quality factor and strongest field enhancement were both observed at the vicinity of quasi-static limit, thereby signifying that the structure exhibited simultaneous optimizations of field enhancement and loss inhibition, which is crucial to the design of high-quality SPP-based devices.
